Wegovy HD vs. Zepbound: A Doctor's Honest Comparison for Weight Loss Plateaus
By Manuel Carrazana, MD, FACP, DABOM — Board-certified in Obesity Medicine and Internal Medicine
If your weight loss on Wegovy has stalled, you now have two FDA-approved paths forward. You can move up to the new higher-dose Wegovy HD (semaglutide 7.2 mg), or you can switch to Zepbound (tirzepatide). Both can work, but the trial data behind them answer different questions. Here's an honest look at what we know, what we don't, and how I help patients choose.
Does a plateau mean Wegovy stopped working?
Not necessarily. Weight loss on any GLP-1 medication naturally slows and levels off over time. That's your body adapting, not the medication failing. Before changing anything, it's worth checking the basics: taking every dose on schedule, eating enough protein, strength training to protect muscle, sleeping well, and reviewing other medications that can promote weight gain.
If those are in place and you still have meaningful health goals to reach, a medication change is a reasonable next step.
What is Wegovy HD?
Wegovy HD is a once-weekly 7.2 mg dose of semaglutide, three times the standard 2.4 mg maintenance dose. The FDA approved it on March 19, 2026, for adults with obesity who have tolerated Wegovy 2.4 mg for at least four weeks and need additional weight loss. It's the same medication you're already taking, just at a higher dose.
How much more weight loss does Wegovy HD provide?
In the STEP UP trial, 1,407 adults with obesity and without diabetes were randomly assigned to 7.2 mg, 2.4 mg, or placebo for 72 weeks. Counting everyone who started, average weight loss was:
18.7% on Wegovy HD (7.2 mg)
15.6% on Wegovy 2.4 mg
3.9% on placebo
That's about 3 percentage points more than the standard dose. About one in three people on 7.2 mg lost at least 25% of their body weight. In people with type 2 diabetes (STEP UP T2D), weight loss on 7.2 mg was 13.2%.
One important caveat: STEP UP started people on the higher dose from the beginning. It didn't specifically study people who had already plateaued on 2.4 mg, so we don't yet know exactly how much extra weight loss to expect in that situation.
How does Zepbound compare with Wegovy?
This is where we have the strongest evidence. SURMOUNT-5 was a head-to-head trial of 751 adults with obesity and without diabetes, each on the highest dose they could tolerate. At 72 weeks:
Zepbound (10 or 15 mg): 20.2% average weight loss
Wegovy (1.7 or 2.4 mg): 13.7% average weight loss
Zepbound also reduced waist size more (7.2 inches vs. 5.1 inches).
Which is better: Wegovy HD or Zepbound?
No trial has compared Wegovy HD directly with Zepbound. SURMOUNT-5 compared Zepbound with standard-dose Wegovy, not the new 7.2 mg dose.
Looking across separate trials, both 72 weeks long and both counting everyone who started, Wegovy HD produced 18.7% weight loss in STEP UP and Zepbound 15 mg produced 20.9% in SURMOUNT-1. Those numbers are in the same general range, with Zepbound modestly ahead on paper. But different trials enroll different people, so this comparison can't settle which medication will work better for you.
How do the side effects compare?
Wegovy HD: Stomach-related side effects were more common on 7.2 mg than on 2.4 mg (70.8% vs. 61.2%). The bigger difference was dysesthesia, meaning unusual skin sensations like tingling, burning, or sensitivity to touch. It affected 22.9% of people on 7.2 mg versus 6.0% on 2.4 mg.
Zepbound: In SURMOUNT-5, fewer people stopped Zepbound because of stomach-related side effects than stopped Wegovy (2.7% vs. 5.6%).
Everyone tolerates these medications differently. If nausea or other side effects have been a struggle on Wegovy, that matters when choosing a next step.
What do Wegovy HD and Zepbound cost?
Insurance coverage varies widely, and many plans cover one of these medications but not the other. For self-pay patients, current manufacturer pricing is:
Wegovy HD: $399/month through NovoCare Pharmacy
Zepbound 7.5 mg to 15 mg: $449/month through LillyDirect, when refilled within 45 days
Eligible Medicare Part D enrollees may be able to get either medication for $50/month through the Medicare GLP-1 Bridge program, which runs through the end of 2027. Prices and programs change often, so confirm current costs before you decide.
How do I decide between Wegovy HD and Zepbound?
In my practice, the choice comes down to your history, your goals, and your coverage.
Stepping up to Wegovy HD may make sense if you:
Tolerate Wegovy well and prefer to stay on a familiar medication
Have established heart disease, since Wegovy is FDA-approved to reduce heart attack and stroke risk in adults with heart disease
Have insurance coverage for Wegovy but not Zepbound
Switching to Zepbound may make sense if you:
Have struggled with stomach-related side effects on Wegovy
Still have a large gap between your current weight and your health goals
Have obstructive sleep apnea, since Zepbound is FDA-approved to treat it
Have better coverage for Zepbound
There's no official formula for converting a Wegovy dose to a Zepbound dose, so switching should be planned with your physician. Either way, a plateau is a signal to reassess, not a reason to give up.
